Abstract

成長期の児童に一定期間のガム咀嚼トレーニングを行うことによって,咬合力および口唇閉鎖力の向上,ならびに咀嚼パターンに変化を生じることが報告されている.本研究は,成長期の児童に対する硬性ガム咀嚼トレーニングが咀嚼運動の変化,上下顎第一大臼歯の近心頬側咬頭間幅径,頬舌的歯軸傾斜角および上顎第一大臼歯口蓋幅径の成長変化に与える影響について調査した.被験者は千葉日本大学第一小学校の44名(男児23名,女児21名)でHellmanの歯齢IIIB~IIICの児童(平均年齢11.2±0.29歳)である.被験者には咀嚼トレーニングとして1日2回朝夕食後の10分間,硬性ガム(DAY-UPオーラルガム<かむトレーニング>,(株)LION,東京)を噛むことを指導した.トレーニング期間は3か月間とし,その後3か月間の経過観察を行った.咀嚼トレーニング開始前(T1)と経過観察終了時(T2)に印象採得および顎機能検査を行い,計測を行った.また,T1の顎機能検査において,咀嚼経路幅が平均値より広いものをwide group(W群),狭いものをnarrow group(N群)とし,両群の変化量の比較を行った.その結果,T1とT2の比較では,咀嚼経路幅が有意に増加し,グラインディングタイプ咀嚼への変化がみられ,上下顎第一大臼歯近心頬側咬頭間幅径,下顎第一大臼歯頬舌的歯軸傾斜角ならびに上顎第一大臼歯口蓋幅径が有意に変化した.またW群とN群のT2における変化量の比較では,上下顎第一大臼歯間幅径,上顎第一大臼歯口蓋幅径および下顎第一大臼歯頬舌的歯軸傾斜角において,N群はW群より有意に増加した.以上の結果から,硬性ガムを使用した咀嚼トレーニングは,グラインディングタイプ咀嚼を誘導し,上下顎第一大臼歯植立に影響を与え,歯列幅径が増加することが認められた.

Masticatory exercise for a set period of time improves maximum bite force and labial-closure strength and changes masticatory patterns. The purpose of this study was to investigate the effects of masticatory exercise with hard chewing gum in children in the growing period on the change in masticatory movement, and the growth changes of dental arch width of the upper and lower first molars, bucco-lingual inclination, and the palatal width of the maxillary first molars. The subjects were 44 students (average age, 11.2±0.29 years) of Chiba Nihon Daigaku Daiichi Primary School (23 boys and 21 girls) of Hellman dental developmental stage IIIB to IIIC. The subjects were instructed to chew hard gum (DAY-UP ORAL GUM <Kamu Training>, LION Corporation, Tokyo) for 10 minutes twice daily, after breakfast and dinner as masticatory exercise. The training period was set as 3 months, followed by a 3-month observation period. We conducted impression taking and jaw function examination and subsequently measured before the masticatory exercise (T1) and after the follow-up (T2). In addition, in the jaw function examination of T1, masticatory path width wider than the average value was set as wide group (W group), and the narrower as narrow group (N group); we compared the change volume in these two groups. In the comparison of T1 and T2, the masticatory path width significantly increased and changed into grinding-type mastication and also dental arch width of the upper and lower first molars, the bucco-lingual inclination of the mandibular first molars, and the palatal width of the maxillary first molars changed significantly. Furthermore, the dental arch width of the upper and lower first molars, the palatal width of the maxillary first molars, and the bucco-lingual inclination of the mandibular first molars more significantly increased in N group than W group. In conclusion, it was demonstrated that masticatory exercise using hard gum leads to grinding-type mastication, gives impact on growth of the upper and lower first molars, and increases dentition width.
