Abstract

少子・核家族世帯の増加率上昇は,家族成員相互のケア能力および子どもの養育能力の脆弱化に関連しているといわれており,さまざまな社会問題を生み出している。本研究はそのような環境下にある母親に対する心理援助の適切な時期について検討する目的で調査を行った。産後12週までの母親91名を対象とし,産後3日,1,2,3,4,8,12週の7時期においてPOMS,STAI,GHQによる縦断的調査を行った。分析は分娩経験と産後時期の2要因について2元配置分散分析,多重比較を行った。T-A(緊張-不安),D(抑うつ-落ち込み),F(疲労),C(混乱),精神健康度は産後3日〜12週にかけて漸減傾向があり,さらに産後2週,4週において変動を示す心理であった。A-H(怒り-敵意)は産後3日〜12週にかけて著しい変動がみられず,V(活気)は産後3週以降12週にかけ上昇するという推移を示していた。また,GHQ,STAIも漸減傾向を示すなか,産後2週および4週に変動を示していた。産後の母親は,産後2週および4週において「緊張-不安」「抑うつ-落ち込み」「怒り-敵意」「疲労」「混乱」の心理が強く現れ,精神健康度が低く不安が強い状態にあるため,この時期においてはより心理的援助が必要であると示唆された。

Purpose: Owing to the declining birthrate and increase in nuclear families, there has been a decline in the caring and child-nurturing abilities of family members. Postpartum women, in particular, find it difficult to care for a child, and this causes them to spiral into depression after childbirth. This study investigates the psychological state of postpartum women to identify the underlying causes of their condition; the study aims to use the findings to develop an ideal method for providing psychological support to such women. Methods: We observed 91 mothers for a period of 12 weeks after childbirth and investigated their psychological states by administering the subjects to three questionnaires-the Profile of Mood States (POMS), State/Trait Anxiety Inventory (STAI), and General Health Questionnaire (GHQ) -seven times over a period of three months after childbirth. We performed a two-way repeated measures analysis of variance, followed by multiple comparisons. Results: A gradual decline was observed in the tension-anxiety (T-A), depression-dejection (D), fatigue (F), confusion (C), and sanity from the third week to the twelfth week after childbirth, and to show a change more in four weeks for two weeks in after giving birth. There was no significant change with regard to anger-hostility (A-H) from the third week to the twelfth week after childbirth, while vigor (V) rose during this period. Discussion: The T-A, D, A-H, F, and C were high; moreover, the psychological states of the postpartum women were apparent particularly two weeks and four weeks after childbirth, and the psychological conditions of the subjects was negative as their sanity levels were low, and it depended. Psychological support needs to be provided during this period, was suggested.
