Abstract

本研究は,非妊時BMIが「標準体重」に該当した妊婦371人を対象者とし,出産に至った過程において,妊娠期間中の母体体重増加量,およびその他関連因子について実態調査し,低出生体重児を出産する要因について成熟児出産群と低出生体重児出産群(2,500g未満の出生体重児)の2群に分類し,検討した。この結果,対象者の低出生体重児出産率は, 5.4%であった。平均年齢は29.7±4.7歳, 2群比較では,低出生体重児出産群が有意に若く(P<0.01),平均在胎週数は39.0週であり,同様に低出生体重児出産群が有意に短かった(P<0.01)。また,母体体重増加量は妊娠中期以降に有意差を認め,低出生体重児出産群のほうが少なく(P<0.01),胎児推定体重は,妊娠初期(P<0.05),および妊娠中期(P<0.01)に有意差を認め,低出生体重児出産群のほうが軽い結果であった。さらに,出生時体重と妊娠各期間における母体体重は,成熟児出産群のみに正の相関を認め,母体体重が増加するに伴い,出生時体重も重くなっていた(P<0.01)。したがって,「標準体重」の妊婦であっても,非妊時BMI別推奨体重増加量を超えないことをめざす栄養指導のみではなく,母体体重増加,および胎児推定体重を考慮した必要栄養素の摂取などにも着目した指導の必要性が示唆された。

In this study, we discover the reasons why babies were born with a low birth-weight from a standpoint of pregnancy weight change. Actual condition survey of weight changes and the related factors was conducted in 371 pregnant women who had a mean age of 29.7±4.7 and normal BMI before their pregnancy. And the statistical analyses were performed between two categories of subjects who had mature babies (group M) and low-weight babies (<2,500g, group L) at birth. The mean gestational age was 39.0 weeks and the birth ratio of low-weight baby was 5.4%. The mean age of group L were significantly younger than group M (P<0.01), and the gestational ages of group L was also short (P<0.01). The members of group M significantly gained body weight more than that of group L in mid and late trimesters of pregnancy (P<0.01). In addition, it appeared that there is a direct proportion between mother and fetal weight at each period of gestation in group M (P<0.01), but group L did not show this relationship. In conclusion, the nutrition guidance of gestation period should consider various factors which are recommended body weight increment from BMI before pregnancy, estimated fetal body weight and pregnancy weight change even when the pregnant women had normal BMI before pregnancy.
