Professional service is unavoidable "information asymmetry", and it is considered to be difficult that the patient does the precise evaluation of the service. This study aims to explore the perception level and the evaluation structure of the professional service under the asymmetric information. Medicines as investigation object were the diabetes, antithrombotic and osteoporosis which involved high-risk in medication. Respectively, questionnaire survey about "medicinal information provision", "information provision of the administration method", "confirmation of patient information", and "satisfaction for the pharmacy services" were conducted. The same questionnaire items were asked to the pharmacist who performed counseling with the patient. The perception levels of the provided degree were collected by a five-point scale. The gap of the perception score among the patient and the pharmacist was examined and multiple regression analyses were performed. As the result, the perception gap between patient and pharmacists were wide in the item of "measures for forgetting the taking of medicine", "adverse drug interactions", "matters that require attention in the life" and "clinical data". The evaluation structures of multiple regression analyses were different between patient and pharmacists greatly each. It was suggested that recognizing such perception gaps and evaluation structures and aiming at the improvement of professional service contributed to rationalization of medication.
