Granulocyte and monocyte apheresis with Adacolumn(GCAP),once a week for 5 weeks, was shown to be very effective on the patients with active ulcerative colitis. In a comparison of patients who have achieved remission and those who have not, the patients treated by steroids for more than 4 weeks or who had a less-than-average total complement in serum were thought to have hardly achieved remission by GCAP. Although there was no difference in regard to clinical backgrounds, laboratory data, and immunological parameters between the patients who had achieved remission and those who had not, the patients with colonoscopic findings of deep ulcers surrounded by pseudopolyposis were thought to need GCAP twice a week. And GCAP once amonth was suggested to be useful for preventing a relapse of ulcerative colitis. The mechanism of efficacy of GCAP with Adacolumn was thought to reveal antiinflammatory effects on ulcerative colitis by pseudoinflammation that occurred by the activation of monocyte and granulocyte in Adacolumn.
