- S021014 Fluid Mechanical Study on the Volume Embolization Ratio for coiling of a Terminal Aneurysm

Coil embolization is a procedure in which metallic coils are deployed in an aneurysm to induce blood flow stagnation to enhance formation of blood clot, whereby preventing rupture and further growth of the aneurysm. The clinical outcome of anuerysmal coil embolization such long-term stability was evaluated with an index called volume embolization ratio (VER) (a ratio of a coil volume to an aneurismal volume). Here we investigated intraaneurysmal blood flow at a variety of VER and a relative positioning of the terminal aneurysm against a parent blood vessel of T-shaped bifurcation in order for discussing effective volume embolization ratio in terms of flow stagnation within an embolized aneurysm. A porous media modeling was employed to describe blood flow within the embolized aneurysm with coils. The results showed a significant variation of blood flow stagnation in the terminal aneurysm in accord with a change in the relative position of the aneurysm. When an aneurysm was relatively less angled against the parent vessel, blood inflow lost its velocity beyond the entrance of the aneurysm under the condition of VER=20%. However, as angled more, blood inflow to the aneurysm did not diminish its velocity and did not well stagnate even with VER of 20-30 %. In conclusion, the guideline value of VER needs to be reconsidered by taking account of the relative position of a developed aneurysm and hemodynamics within the aneurysm.
