Abstract

本研究の目的は,看護師が養護教諭と連携して,小学校5年生を対象に食習慣に焦点をあてて実施した「学童とその親の生活習慣改善プログラム」の効果と問題点を明らかにすることである.プログラムは,対象校の調査データに基づく生活習慣上の問題や,学習進度,学校内の協力体制などを考慮して計画した.資料等の準備および評価は看謹師が申心に行い,具体的なプログラムの実施は養護教諭・担任教員・栄養士が行った.プログラムの評価は,プログラム実施前,終了時,終了1年後の3時点で生活調査を実施・回収できた82名(男子47名,女子35名)のデータを分析した.プログラム終了時には,好き嫌いの減少,脂肪の摂りすぎの減少,夕食時間が早くなる,近くに出かけるときには歩く,便秘の減少がみられた.しかし,終了1年後では,好き嫌いや夕食時聞は実施前と有意差が無くなっていた.プログラム実施前と比較してプログラム終了1年後の肥満度は男女共に有意な低下がみられた.プログラム実施後に肥満が改善した学童4名は,肥満が改善しなかった5名と比較し,プログラム実施前の肥満度が軽度である,身長の伸びが大きい,生活習慣上の摺題が少なくプログラム後に食習慣の改善が持続している,という特徴がみられた.

The present study involved a nurse and school nurse working cooperatively to clarify the effects and limitations of a program focusing on the eating habits of elementary school fifth year pupils. The program was planned based on investigations at the school regarding learning achievement and eating habits. The school nurse. class teachers and school dietician performed the actual program. The nurse was involved in preparation of materials and evaluation of the program. The program analyzed 82 data recovered from 3 time points:before initiating the program, at completion of the program, and at l year after program completion. At completion of the program, improvements in daily Iife habits were 0bserved, including increased walking, decreased constipation and unbalanced diets, and earlier timing of the evening meal. However, improvements in unbalanced diet and time of evening meal were not maintained after l year. Body fatness of children after l year was significantly lower than that before initiating the program. Obese children whose body fatness had decreased to <20% after 1 year displayed the following characteristics: body fatness of <31% before program initiation; 1arge increases in stature; presence of few problematic lifestyle habits; and improved eating habits were continued after completion of the program. Execution and evaluation of the program by the nurse in cooperation with the school nurse enabled that the consideration was deepened on effect of the prograrn.
