結核病棟看護師の看護実践の特徴

Abstract

本研究は、結核病棟の看護師が実践する看護の特徴を明らかにすることを目的とした。大阪府内の結核病棟に勤務する看護師11名を対象とし、実践している看護や、工夫していること、困難なことなどについて半構造化面接を実施し、質的帰納的分析を行った。分析の結果、【服薬支援】、【患者指導】、【精神的支援】、【地域や他職種との連携】、【ソーシャルサポート】、【退院後の生活に向けての援助】、【結核患者特有の難しさへの対応】という7つのカテゴリーが抽出された。結核病棟の看護師は、服薬支援を重要と考え、療養の指導を行っていた。また患者を精神的に支え、退院後も治療が継続できるように地域との連携を行っていた。しかし、ストレスの高い患者の対応などに対しては看護の難しさや困難さを感じている現状が明らかになった。今後、隔離入院している患者のストレス緩和などを検討する必要がある。

The purpose of the present study was to clarify the characteristics of nursing practices by nurses in the tuberculosis ward. Semi-structured interviews regarding nursing practices, planning, difficulties, etc. were carried out among 11 nurses working at the tuberculosis ward in Osaka Prefecture as the subjects, after which qualitative and inductive analyses were carried out. Upon analysis, the following seven categories were extracted:［medication administration support］;［patient education］;［mental support］;［cooperation with the community and other professionals］;［social support］;［assisting patients with their life following discharge from hospital］; and［dealing with difficulties unique to tuberculosis patients］. Nurses in the tuberculosis ward believed that providing medication administration support was most important and provided education on medical treatment. Moreover, they mentally supported patients and cooperated with the community in order that patients could continue treatment following discharge from hospital. However, it was found that nurses experience various difficulties and challenges when dealing with patients demonstrating high stress levels, etc. Going forward, it will be necessary to relieve the stress of such isolated hospitalized patients.
