[要旨] 近年，CPG（Clinical Practice Guidelines）の開発は，GRADE（Grading of Recommendations Assessment,Development and Evaluation）システムが活用されてきている．GRADEシステムのアプローチによって、エビデンス評価の精緻化が進むと共に，臨床家の経験，患者の視点・価値観を考慮した総意形成が重要視され，“evidence-based consensus guidelines”の形が確立しつつある．これは，医療の質の向上に向けた努力であり，科学的な信頼性と妥当性を高めるCPG開発とCPGの臨床活用により推進されることから，看護実践におけるCPGの開発と位置づけが重要となってきている．このたび，看護国際化ガイドライン開発に向けて，2017キックオフ国際シンポジウム「診療ガイドラインの軌跡と未来形」を企画開催したので，実施内容について報告する．看護実践の中心的概念に患者と家族中心のケア（P&FCC:Patient and Family Centered Care）があり，患者の価値観，希望に十分に配慮した患者中心のケアの提供と，多様性に配慮した文化的ケアのための組織づくりが進められている．医療の国際展開が進む中，看護職を含めた医療職，患者，社会の協働と信頼関係の構築に看護職がプロフェッションとして参画するために，CPGの開発とCPGを看護実践の拠って立つところの一つとして位置づけることの重要性が示された．

[Abstract] In recent years CPG (Clinical Practice Guidelines) were developed using the GRADE (Grading of Recommendations Assessment, Development and Evaluation) system. The elaboration and sophistication of the Grade system have been improved to evaluate the evidence of research outcomes, as well as to reach consensus between a practitioner’s experience and a patient’s viewpoint and preferences. The system stimulates the establishment of“evidence-based consensus guidelines.”These efforts are milestones in reaching the goal of improved health care quality. To develop trustworthy guidelines and implement them in nursing practice, value judgement of CPG in nursing practice is important. The 2017 international symposium,“Trajectory and Future of CPG,”was held as a kickoff for the new research project to develop guidelines of grobal health and nursing. This paper provides a summary of this symposium and discusses value judgment of CPG in nursing practice. A core concept in nursing practice is P&FCC (patient and family centered care). Hence, nursing practice provides nursing care with consideration for patients’perspectives and values, and an organizational culture of culturally competent care for diversity in the healthcare arena has been established. Creating trustworthy CPG involves healthcare providers, patients and family, the community, and society. This symposium makes it clear that development and value judgement of CPG in nursing practice is a key to the future agenda of CPG.
