Abstract

PTC施行時に肝動脈を破り,これからの胆管内出血が契機となって広範性肝壊死をおこした稀な症例を報告した.広範性肝壊死の発生機序にShwartzman現象が関与していると考えられた.すなわち,胆石症に続発した胆管炎が準備注射にあたり,胆道内出血による胆道内圧上昇が腸内細菌およびェンドトキシンの血中侵入を促したことが惹起注射に相当するものと考えられた.

A rare fatal case as a complication of percutaneous transhepatic cholangiography (PTC) was reported. Massive hepatic necrosis resulted from bleeding into the bile duct by a puncture of the hepatic artery branch developed following PTC. It was thought that Shwartzman phenomenon is involved in the pathogenesis of massive hepatic necrosis. In a word, bacterial infection to bile stasis and bacterial reflux from bile ducts into systemic circulation resulted from an increased biliary pressure following bleeding from the hepatic artery branch correspond to a preparative and provocative injection, respectively.
