Hepatocellular carcinoma (HCC) often recurs even if the primary hepatic resection was curative. HCC can recur because of intrahepatic metastases or multicentric occurrence. We analyzed nodules in noncancerous regions of 161 specimens of resected HCC obtained in the past 5 years. In 31 cases (19%), 48 nodules were detected that were not found before or during surgery. Nineteen (40%) were large regenerative nodules, ten (21%) were of adenomatous hyperplasia, eight (17%) were of early HCC, and eight (17%) were of intrahepatic metastases. Now, if recurrences of HCC are to be prevented, a method for detection of these lesions before surgery must be developed. The therapy will depend on the kind of recurrence.
