抄録

Acetoaminophen-overdose is generally known to result in acute liver injury. In two cases, who were 38-year-old man and 31-year-old woman, severe liver injury caused by ingesting small dose of acetoaminophen was observed and they were relieved. After ingesting 3.7 and 6.4g of acetoaminophen respectively, they were hospitalized because of nausea, abdominal pain and general fatigue.<BR>In male case, endocarditis, renal dysfunction and acute pancreatitis were noted in addition to markedly increased serum transaminase accompanied with hyperbilirubinemia. Liver biopsy specimen showed the focal necrosis and thickness around the central vein.<BR>In female case, not only severe liver dysfunction but thrombocytopenia were shown. Her liver revealed severe bridging necrosis.<BR>In conclusion, the reason why small dose of acetoaminophen induced the severe liver injury will be due to special backgrounds in these cases, such as a fasted state and alcohol-intake.
