Hepatitis B virus (HBV) DNA in hepatocytes was visualized by an in situ hybridization method using 3H-labelled probe. HBV DNA was visualized as silver grains on 47 of 63 liver specimens from patients with chronic hepatitis B. The distribution of HBV DNA positive hepatocytes was classified into three patterns: diffuse (HBV DNA distributed diffusely within the section), lobular (HBV DAN was present in 1/3-2/3 of the section) and spotty. Nineteen of 47 specimens showed diffuse, 17 showed lobular and 11 showed spotty pattern. Most cases of diffuse and lobular pattern were serum HBeAg-positive and HBV DNA polymerase activity-positive. Immunohistochemical study of HBsAg and HBcAg in serial sections revealed that HBcAg positive hepatocytes were usually HBV DNA rich, although HBsAg positive hepatocytes were not always HBV DNA rich. These data indicate that HBV DNA detected by in situ hybridization is a useful marker for HBV replication.
