抄録

To improve the safety of cardiopulmonary bypass, the following several trials were made in this institution. (1) For the purpose to prevent the platelet aggregation during cardiopulmonary bypass, gabexate-mesilate was given, but indefinite conclusion was drawn from the change of platelet count and β-thromboglogulin value. (2) In order to improve the glucose metabolism avoiding excessive hyperglycemia in diabetic patients, concentration of glucose in GIk solution was decreased from 2:1:4 to 1:1:4 This modified GIK solution was very effective to maintain the adequate level of blood glucose, preventing abnormal increase in free fatty acid. (3) In order to decrease the hemolysis during cardiopulmonary bypass, single RA-IVC cannula was preffed in case of AVR and AC bypass. This cannula wass superior to separate SVC and IVC cannulation to secure venous return to heart-lung machine, and no need to open the right atrium, resulting to reduce hemolysis. (4) ECUM was often used to maintain water balance during cardiopulmonary bypass and to remove excessive water from the primed blood after completion of the bypass. Use of ECUM was effective to elevate hematocrit value and resulted in saving the blood of 5 units in average.
