Abstract

目的:現代,中年期以降の女性は従来とは異なるストレス環境下にある.ストレスや心身の不調を訴える者は増えているがストレス緩和やその実践方法に関する研究は少ない.このことから自分に合ったストレス緩和法を見つけ,実施できるようになる教育が必要である.そこで,本研究では自身でできるストレス緩和法(アロマセラピー,有酸素運動,漸進的筋弛緩法)を3か月間実施してもらい,実施前後のストレス指標の変化を検討した.<br>方法:35歳以上の女性46名を対象とした自己選択によるストレス緩和法を用いた介入を行った.すなわち,対象者をストレス症状の程度や本人の希望を参考にアロマセラピー群(n=12),有酸素運動群(n=12),漸進的筋弛緩法群(n=12)及び対照群(n=10)に分けた.各々の緩和法は3か月間実施され,群間及び介入前後の諸指標の変化を比較した.評価は,簡略更年期指数(以下;SMI),Quality of Life SF 36v.2(以下;QOL),心理的ストレス反応尺度(以下;SRS-18),血液より白血球数,血小板数,好中球数,リンパ球数,単球数,唾液よりクロモグラニンAを用いた.<br>結果:対象者の各緩和法の実施状況はアロマセラピー群の1名を除いて実施条件を満たしていた.SMIは筋弛緩群で介入により有意に低下した.QOLはアロマセラピー群で「全体的健康感」と「社会生活機能」,有酸素運動群で「身体の痛み」,「活力」,「日常役割機能・精神」,「心の健康」が介入前より介入後で改善されていた.SRS-18 は有酸素運動群で「無気力」が介入によって低下していた.血液中のリンパ球数が有酸素運動群で介入後に減少していた.<br>結論:3か月間の緩和療法によって更年期症状やQOLは改善され,特に有酸素運動群,アロマセラピー群でその傾向が強かった.ただ,嗜好の個人差もあり,自身に合った方法を選択・実践することも必要であると示唆された.

Purpose: This study examined the effects of stress relaxation interventions for 3 months to compare the methods of aromatherapy, exercise, and muscle relaxation.<br>Methods: In this intervention study, participants were 46 women aged 35 and older. They were divided into the aromatherapy (n = 12), aerobics exercise (n = 12), muscle relaxation (n = 12), and control groups (n = 10). Each treatment was carried out for 3 months; the control group did not receive any therapy. A simple menopausal index (SMI), Medical Outcomes Study 36-Item Short-Form Health Survey (quality of life; SF36 v.2) scale, and stress response scale (SRS-18) were administered before and after the interventions. In addition, serum levels of white blood cells, blood platelets, neutrophils, lymphocytes, and monocytes were measured, and levels of Chromogranin A in saliva were measured before and after the interventions.<br>Results: Almost all the participants of intervenient groups complied with the stress relaxation methods. The muscle relaxation group showed significant decreases on the SMI scale after the intervention. For quality of life, “general health perception” and “social functioning” improved significantly in the aromatherapy group, and “bodily pain,” “vitality,” “role emotional,” and “mental health” improved significantly in the aerobics exercise group. Improvements on the SRS-18 were also seen in the aerobics exercise group, although differences did not reach statistical significance. Lymphocyte numbers, an indicator of stress, decreased significantly in the aerobics exercise group.<br>Conclusion: Stress relief treatment for 3 months improved menopausal symptoms and quality of life, especially in the aerobics exercise and aromatherapy groups. However, because individual variations were large, they should be permitted to choose and practice the most suitable stress-relieving method individually.
