経仙骨的に摘除した直腸原発奇形腫の1例 A CASE OF TRANSSACRALLY EXCISED TERATOMA ORIGINATING IN THE RECTUM

Abstract

65歳の女性で便秘を主訴に来院し,注腸X線透視で直腸に円形の石灰化陰影を含んだ楕円形の腫瘤陰影を認め,大腸内視鏡検査では表面に毛髪を有する直腸腔内を満たす腫瘤を認めた.経仙骨的に腫瘤を摘出したところ,大きさ8×6×4cmで,表面に毛髪を有し,割面では骨組織,嚢胞やメラニン色素沈着がみられた.病理組織学的に成熟型奇形腫と診断した.直腸原発奇形腫は非常に稀で,現在までに40例が集計されているにすぎない.経仙骨的直腸切開術にて摘除したのは本症例が初めてと思われるので,文献的考察を加えて報告した.

A 65-year-old woman visited our hospital with the chief complaint of constipation. Barium enema examination revealed an oval tumor mass containing a round shadow of calcification in the rectum. Colorectal endoscopy also showed a tumor with hairs on the surface occupying the rectal space. The tumor, which was transsacrally excised, was 8×6×4 cm in size, had hairs on the surface, and presented osseous tissue, cysts and melanism on the cut surface. Histopathologically it was diagnosed as mature teratoma. Rectal teratoma is rare with only 40 cases having been reported to date. In the present paper, the first case known to be managed by transsacral excision is reported together tiwht a review of the literature.
