A 55-year-old man who had been found to have a submucosal tumor of the stomach 15 years before complained of epigastralgia in July, 1998. He consulted his family doctor and was referred to the hospital with a diagnosis of early gastric cancer and a submucosal tumor. After admission an early gastric cancer at the lower body and a solid submucosal tumor at the antrum were confirmed. Distal gastrectomy with lymph node dissection (D2) and reconstruction with jejununal inter-position was performed on January 8, 1999. It was reported that the depth of cancer was mucosa and no lymph node metastasis was seen by pathological examination. On gross examination, the excised tumor was a submucosal solid tumor covered with normal mucosa which was composed of spindel cells. Tumor cells showed positive staining for vimentin and CD-34 but negative for Desmin, α-smooth muscle actin and S-100 protein. The expressions of these markers represented neither typical leiomyoma nor schwannoma and the tumor was diagnosed as gastrointestinal stromal tumor without apparent differentiation. It was unclear why both early gastric cancer and gastrointestinal stromal tumor occurred simultaneously, but it seemed to be extremely rare.
