A series of 49 patients with carcinoma of the papilla of Vater treated by pancreaticoduodenectomy with lymph node dissection were subjected to a clinicopathological study, in order to elucidate prognostic factors for the improvement of surgical outcomes. And the appropriate lymph node dissection was evaluated. The cumulative 5-year survival rate was 57.1%. Univariate and multivariate analyses revealed that significant prognostic factor were invasion to the pancreas, invasion to the duodenum, lymph node metastasis and gross appearance of the tumor. The ulcer forming tumor showed a higher frequency of invading the pancreas and duodenum, and shorter survival than the tumor forming.<br> It is suggested that in traeating carcinoma of the papilla og Vater, pylorus preserving pancreaticoduodenectomy with D2 lymph node dissection emphasizing the No14 lymph node should be the standard procedure.
