Gastrointestinal stromal tumor (GIST) of the rectum is relatively rare. We report a case of GIST uncommitted type of the rectum together with a review of the literature. An 85-year-old woman was admitted to the hospital because of melena. Digital examination and a magnetic resonance imaging (MRI) scan revealed a hard tumor in the anterior wall of the lower rectum. We performed a Miles' operation with a hysterectomy, a bilateral salpingoophorectomy and a resection of the posterior wall of the vagina under a diagnosis of rectal submucosal tumor (SMT). The tumor was 8.0×7.0cm in size and looked to be SMT. Histologically, the tumor was composed of spindle shaped cells and an immunohistochemical study showed that the tumor cells were negative for SMA and S-100 and positive for CD34 and KIT. Therefore, we diagnosed this tumor as GIST uncommitted type.
