A 56-year-old man who had a lasting anal pain which first appeared at the beginning of January 2001 was reffered to the hospital for close exploration. Close exploration disclosed a giant intrapelvic tumor 10cm in diameter which showed an extrinsic pressure on the lower rectum left backward. Needle biopsy per anus was performed and gastrointestinal stromal tumor (GIST) was suspected. Laparotomy was performed and benign schwannoma was diagnosed by an intraoperative frozen section diagnosis. The tumor itself were comparatively easily dissected from the upper rectum, prostate, and seminal vesicle. But there was a part in the right anterior wall of the lower rectum which resisted to be dissected and necessitiated a transmural partial resection of the rectum. On a postoperative histologic exploration, spindle cells revealed irregular fascicular proliferation on a HE staining. Considering these results as well as those of immunological stainings which were positive for KIT, GIST, uncommitted type having a malignant potential was diagnosed. There have no signs of recurrence to date, but a strict follow-up would be required.
