In a recent 9-year period from 1991, four cases of duodenal carcinoid were treated in our department. These patients were from 70 to 81 years old, with a mean of 74, and these were two men and two women. Chief complaint was upper abdominal discomfort in one patient and the other three patients were detected having a tumor by a medical examination. The tumors were located at the lst portion in three cases and the 2nd portion of duodenum in one case, and showed macroscopially Yamada I type polyp in two cases, and Yamada II type and I (Protruted) type in each one case. Operative procedures included partial resection of the duodenum for two cases, EMR and PpPD for each one case.
As of the longest of 10 years after the operation, all four patients have been free from any recurrence signs and are in good health.
About the choice of operative mode, it is thought from the bibliographical comments that carcinoid of the duodenum which are less than 20mm in size and submucosal or shallower in tumor invasion are indicated local resection and if they are less than 10mm in size, they are indicated endscopic resection with enough informed consent and postoperative clinical observation, but the cases which don't meet such condition or invade into propria muscle or lymphatic vessels are indicated the same radical resection as duodenal cancer.
