A 70-year-old Japanese man with pyloric stenosis caused by unresectable gastric cancer had undergone Roux en Y bypass surgery at the age of 69. Oral anticancer agents had been administered on an outpatient basis. Rectal stenosis caused by Schnitzler's metastasis developed 13 months after the bypass surgery. We inserted a self-expandable metallic stent (EMS) with the flared part located on the oral side in order to prevent migration. The patient was able to maintain bowel activity without stent dislocation for 6 months until the small intestine became obstructed. Despite a laparotomy, conventional colostomy tended to be effective palliation only for a short period, because multiple obstructions occurred later on. EMS placement, which is less invasive compared to colostomy and makes spontaneous bowel movement possible, promised to relieve mental and physical stress of patients in the terminal stage.
