Abstract

近年開発された自動塗抹装置ThinPrep Processor (Cytyc, USA) による子宮頸部の細胞標本 (ThinPrep標本) にっいて, 再現性・易観察性・判定精度を検討した.ThinPrep標本は, スパーテルで擦過採取された細胞を均一に分散する浮遊液とし, その一部を抽出して作製される.同一浮遊液から作られた連続10枚の各標本で, 上皮細胞の数・配列・面積分布, 異型細胞の数・出現頻度および判定結果はほぼ同一となり, この標本作製法が良い再現性を与えるrandom samplingに基づいていることが示された.<BR>またこの標本を用いるとscreeningの省力化が可能であり, 従来の用手塗抹標本に比べ, 25%の塗抹面積, 12%の上皮細胞数, 62%の所要時間の観察で, ほぼ同様の判定結果が得られた.ThinPrep法は, 普及のためには細胞判定基準の確立・完全自動化の工夫・費用の改善など克服すべき問題も少なくないが, 優れた合理性と有用性を備えていると考えられた.

The ThinPrep process, a new automated fluid-based technique for the collection and preparation of cytologic specimens, was evaluated for cervical cytology in terms of its reproducibility, ease of screening and diagnostic sensitivity. A series of 10 consecutive ThinPrep slides made from the same cell suspension, from one patient, provided almost the same number of epithelial cells, pattern of cell arrangement, cell size destribution and number of atypical cells from slide to slide.In comparison with the conventional method of manually smearing slides, the ThinPrep slide screening was revealed to be much more labor-saving, because only 25% of the screening area, 12% observation of epithelial cells and 62% of the screening time were required to obtain the same diagnostic sensitivity.Thus, the ThinPrep process showed its principle of reproducible random sampling and good diagnostic efficacy despite some problems needing improvement, including incomplete automation and high cost performance.
