Abstract

平成10年7月25日和歌山市園部地区の夏祭り会場で出されたカレーに砒素が混入された事件の被災者で急性期に皮疹が認められた3名について報告した。症例1: 31歳男性。第6病日に肝機能異常の出現と同時に両腋窩, 両股部, 下腹部に点状赤褐色丘疹が左右対称性に集籏して認められた。病理組織学的に血管周囲に稠密なリンパ球の浸潤があったが汗腺などの付属器には異常は認められなかった。症例2: 17歳男性。第6病日に肝機能異常の出現と同時に両大腿, 両肘窩に左右対称性に症例1と同様の皮疹が認められた。組織学的にも症例1と同様であった。症例3: 48歳男。第12病日より四肢末端の葉状落屑が認められた。砒素暴露3ヵ月後の23名の検診時の問診から, 急性期にみられた皮疹は紅色丘疹を示した症例が6名, 顔面浮腫3名, 水痘様皮疹が3名, 指趾尖の落屑3名, 紅皮症が1名であり, 経口亜砒酸推定摂取量との関連性はみられなかった。(皮膚, 41: 511-517, 1999)

The curry and rice served at a summer festival site at Sonobe, Wakayama on July 25, 1998 was mixed with arsenite for the purpose of crime and sixty-seven persons sufferedfrom acute arsenic poisoning. We report here on three cases who exhibited rash in anacute stage. Case 1: A 31-year-old male. On disease day 6, hepatic dysfunction occurredwith simultaneous manifestation of discreate brownish red papules grouping symmetricallyin the bilateral axillary and crural areas and lower abdomen. Histopathologically, markedperivascular lymphoid cell infiltration was observed but no abnormality of the sweatorgans. Case 2: A 17-year-old male. On disease day 6, hepatic dysfunction occurred withsimultaneous rash, similar to the one in case 1, distributed symmetrically in the bilateralthighs and cubital areas. Histopathological findings were the same as those in case 1. Case3: A 48-year-old male. On disease day 12, foliaceous desquamation on the acral areaswas exhibited. Examination of 23 patients 3 months after exposure to arsenic revealedthat rash observed in the acute stage included erythematous papules on the intertrigiousareas in 6 patients, facial edema in 3, varicella-like rash in 3, desquamation of finger andtoe tips in 3, and erythroderma in 1. No relationship with predictive oral doses of arsenitewas identified among the patients who exhibited rash.Skin Research, 41: 511-517, 1999
