抄録

A comparative study between adenocarcinoma and squamous cell carcinoma of the lung on clinicopathological factors was performed in relation to hematogenous metastasis. The subjects consisted of 115 patients with adenocarcinoma and 82 patients with squamous cell carcinoma who underwent absolutely or relatively curative resection. Patients with direct growth into adjacent structures were excluded.<BR>During the period of 3 or more years postoperatively, 45% of the patients with adenocarcinoma and 33% of the patients with squamous cell carcinoma relapsed with hematogenous metastasis.<BR>Concerning the factors that related to metastasis, a clear difference between adenocarcinoma and squamous cell carcinoma was observed. In the former, the metastasis developed in 31% of pN0 patients, 76% of pN1 patients and 57% of pN2 patients. In squamous cell carcinoma, the rate was 27%, 24%, and 59%, respectively. Comparing the pNl patients with adenocarcinoma and those with squamous cell carcinoma, the former had a significantly higher rate of metastasis than the latter, while there was no significant difference between adenocarcinoma and squamous cell carcinoma in pN0 and pN2 patients. Cases of adenocarcinoma also showed positive correlation between metastasis and the following factors: tumor size, p factor, cellular atypism, and histological findings in the primary tumor including vascular invasion, hyalinization and mitosis. In contrast, cases of squamous cell carcinoma did not show any such correlation.<BR>These results suggest that there are different factors influencing hematogenous metastasis in adenocarcinoma and squamous cell carcinoma of the lung.
