The important role of the rotator cuff in elevation of the arm is well known. However, the degree of dysfunction of the shoulder is not proportionate to that of rotator cuff tears, and the mechanism of tears remains largely obscure. In this study we measured ball roll (B) and gliding (G), a part of the glenohumeral rhythm, in patients with a rotator cuff tear and compared them with those in healthy controls.<BR>The subjects consisted of 21 patients with a complete tear of the rotater cuff (RCT) and 60 healthy controls (N) . Using an image analyzing system, B and G values during active elevation of the arm on the scapular plane were measured, and the |G|/B ratio and the B+G value were calculated. In addition, evaluation was made in 4 phases of motion (setting phase, hanging joint phase, transitional zone phase, and phase of joint needing support) .<BR>The results were as follows:<BR>1) The B and G values in Group RCT were lower than those in Group N between 0 and 80 degrees but tended to be higher thereafter. The G values varied most widely between 0 and 10 degrees.<BR>B and G decreased in Group N as elevation advanced through the phases. However, in Group RCT, B and G decreased in the hanging joint phase but increased in the phases that followed.<BR>2) Based on the |G| /B ratios and B + G values, the glenohumeral movement in Group N could be divided into 3 phases (first phase 0°-30°, phase of ball roll predominance ; second phase 30°-90°, stable phase; and third phase 90°- 150°, unstable phase) . In Group RCT, no regularity was observed.<BR>These results suggest a great contribution of the rotator cuff, especially the supraspinatus, as a factor regulating glenohumeral rhythm.
