食道病変の生検で肉芽腫を認めたアフタ様病変のみのクローン病の1例 CROHN'S DISEASE WITH ONLY APHTHOID LESIONS DIAGNOSED BY BIOPSY SPESIMENS OF AN ESOPHAGEAL EROSION, REPORT OF A CASE

Abstract

症例は18歳,男性.肛門周囲膿瘍の手術後より,発熱,粘血便,関節痛が続き精査目的で入院した.全消化管にアフタ様病変を認め,食道の生検で粘膜下に非乾酪性類上皮肉芽腫を検出しクローン病(以下CD)と診断した.食道病変を伴うCDはまれであるため本邦報告例の検索を加え報告した.またCDにおけるアフタ様病変の意義について若干の考察を加えた.

A 18-year-old man was admitted to our hospital because of fever, bloody stool and arthralgia after operation of perianal abscesses. On endoscopic examinations, aphthoid lesions were seen in the esophagus, stomach, duodenum, terminal ileum, and colon. Noncaseating epithelioid granulomas were found on the biopsy specimens of the esophageal erosion. Theraby, he was diagnosed as Crohn's disease (CD). CD of the esophagus is a rare process. Cases of esophageal CD in Japan were reviewed, and significance of aphthoid lesions was evaluated.
