Fatigable muscle weakness is the clinical characteristic of the autoimmune disease myasthenia gravis (MG). Dysphagia is a frequent symptom in MG; however, it is uncommon for dysphagia to be the sole or main symptom of the disease. Here, we report an elderly female case with MG presenting with dysphagia as the main manifestation. A 72-year-old female patient was hospitalized for progressive dysphagia. Clinical examination revealed unilateral ptosis and nasopharyngeal incompetence. Videoendoscopic and videofluorographic study demonstrated highly disturbed hypopharyngeal clearance of the bolus which led to marked aspiration. Pharyngoesophageal manometric examination showed obvious weakness of pharyngeal contraction. Serum examination showed elevated anti-acetylcholineesterase antibody. Administration of tensilon immediately relieved symptoms including dysphagia and nasopharyngeal incompetence. With these findings, MG was diagnosed. Clinical features and treatment modalities in elderly cases of MG are discussed.
