Renal biopsies from 21 patients with idiopathic nephrotic syndrome were investigated by immunofluorescence study. IgM was found on the glomeruli of the 18 patients. Six cases out of the 18 patients were examined in regard to the glomerular localization of IgM by electron immunoperoxidase method. The IgM deposits were found mainly in the subendothelial area of the mesangium. One patient with prolif erative change showed the deposits of IgM in the site of not only subendothelium but also basement membrane and subepithelium. Spontaneous remission took place in this case. Pathological significance of IgM deposits in the nephrotic syndrome is in dispute. We think that the IgM deposits in the mesangium may be due to rather uptake of macromolecules than some immunological mechanisms.
