Allergic response to the dental prosthetic metals has been considered to be one of the causative factors of oral lichen planus (OLP), however, no evidence has been reported. Therefore, we performed the following experiments in order to clarify the participation of the dental prosthetic metals to immunological mechanism in the development of OLP:<BR>(1) skin patch tests of dental metals, <BR>(2) analyses of the components in the dental prosthetic metals by X-ray microanalysis, and<BR>(3) analyses of the elemental metals localized in the mucosal lesions of OLP by X-ray microanalysis.<BR>In the present study, we found the following evidences:<BR>(1) in OLP patients (31 cases), the same elemental metals showing a positive response to the skin patch tests (21 cases) were significantly detected in each prosthesis (7 cases), (2) inOLPmucosallesions, the same elemental metals (Co) were located in the lysozome of keratinocytes which expressed HLADR antigen (1 case), and (3) HLA-DR positive dendritic cells were observed adjacent to the keratinocytes.<BR>Based on these findings, it was reasonable to be consider that metal ions which were derived from the dental prosthetic metals accumulated in the lysozome of keratinocytes, and keratinocytes and dendritic cells as antigen presenting cells (APC) were related to the allergic response to the dental metals.<BR>These results strongly suggest that the dental metals is a causative factor of OLP.
