We report the case of an infant with Evans syndrome. She was admitted to our hospital with complaints of pyrexia, anemia and thrombocytopenia at 6 months of age. She developed hemolytic anemia, however she was negative for both direct antiglobulin and cold-reactive antibodies. We subsequently confirmed the diagnosis of Evans syndrome by measuring the amount of surface immune globulins on red blood cells (RBC-associated IgG) and the bone marrow findings, i.e. increase of megakaryocytes (816/μl) and absence of platelet production. PAIgG remained normal and anti-platelet antibody was negative. It is important to measure the surface immunoglobulins on RBC surface in patients with hemolytic anemia who show a negative Coombs test.
