Abstract
It is important in the treatment of patients to ensure satisfactory coordination between the local base hospitals and clinics to achieve excellent regional medical service. In recent years, a high curative effect has been obtained with the use of biologics for psoriasis, one of the chronic skin diseases. We conducted a questionnaire survey of the doctors in Nagano prefecture (hospitals and clinics with a department of dermatology) to obtain a clear picture of the current state of treatment of psoriasis. We mailed the questionnaire to 159 establishments to obtain information on the number of patients at each of the establishments, the current treatment employed, and the status of understanding of each doctor regarding the usefulness of biological drugs, etc. We obtained responses from 87 of the establishments (response rate, 55 %). Analysis of the results of the survey revealed that there are about 3000 persons afflicted with psoriasis in Nagano prefecture, and that about 70% of these patients are treated at the clinics. More than 80% of patients are treated with topical preparations. Our analysis also revealed that there were 34 persons in 15 establishments who needed treatment with biologics. The results also revealed that among the factors that hamper the start of treatment with biologics at the clinics was the fact that the doctors have no time to explain the treatment with biologics to the patients and the concern of the doctors about the adverse effects of these drugs. We concluded from the survey that cooperation between clinics and the base hospitals is important in the medical management of psoriasis using biologics.
