抄録

Two cases of factitious anemia induced by self-blood letting are reported. Both patients were unmarried nurses. Hypochromic anemia had been progressive in spite of adequate iron therapy, but source of hemorrhage was not found out. Besides iron deficiency anemia, hypoproteinemia with slight hypogammaglobulin was marked in both cases. In the first case transient red cell aplasia and occasionally urine hemosiderin was observed, but administration of steroid and Endoxan had little effect. Massive transfusion was repeated before accurate diagnosis was made. When hemoglobin was checked every day, an unreasonably decreasing curve was noticed. A careful watch over the patient disclosed a scar of the venipuncture, and her aunt who had been living with the patient had found needles, syringes and a bottle of blood in her room. In the other case diagnosis was made rather quickly by the same observation. Psychiatric aspects are discussed in both cases. After careful consultation and confrontation, anemia recovered rapidly. To our knowledge, this paper is the first report of factitious anemia in Japan.
