- IgD-Type Multiple Myeloma Associated with Tumor of the Pancreas

A 71 year-old male was admitted to the hospital because of upper abdominal discomfort. Laboratry examination revealed monoclonal gammopathy with increase in IgD (650 mg/dl). M-protein was identified as IgD-type using electrophoresis. IgG, IgA, and IgM levels were markedly decreased. Bone marrow aspiration revealed 84% atypical plasma cells. Serum and urine amylase were elevated to 31,070 IU/l and 21,512 IU/l, respectively. Both amylase isozyme patterns showed salivary (S) type. A tumor of the pancreas was confirmed by abdominal ultrasonography, CT scanning, endoscopic retrograde cholangiopancreatography and angiography. The tumor became undetectable after treatment with cyclophosphamide, vincristine, melphalan, and prednisolone. Serum and urine amylase also decreased. Thus, amylase was thought to be derived from myeloma, not from a tumor of the pancreas.
