Abstract

オルビフロキサシンは濃度依存的に殺菌活性を示す動物用フルオロキノロン系抗菌薬であり,おもに腎臓から排泄される.本研究では,慢性腎臓病(Chronic Kidney Disease : CKD)モデル犬及び健常犬にオルビフロキサシンを5.0mg/kg単回経口投与し,血中濃度を経時的に測定した.CKD群(n=3)及び健常群(n=3)の糸球体濾過量はそれぞれ2.32±0.45m<i>l</i>/min/kg,4.14±0.66m<i>l</i>/min/kg であった.オルビフロキサシンの最高血中濃度は,健常群で投与1時間後に3.60±0.23μg/m<i>l</i>,CKD群で投与2時間後に3.07±0.31μg/m<i>l </i>であった.平均血中濃度は投与24時間後に健常群では0.45±0.14μg/m<i>l</i>,CKD群では0.41±0.10μg/m<i>l</i>であった.両群ともにオルビフロキサシンの平均血中濃度及び7日間連続経口投与シミュレーションによる血中濃度推移に有意差は認められなかった.本研究より,オルビフロキサシンは腎機能の低下したCKDの犬において,投与量及び投与間隔の調整は不要であるものと示唆された.

Orbifloxacin, is a fluoroquinolone antibiotic developed for use in veterinary medicine that is quickly and widely distributed after administration and is excreted primarily by kidney. In this study the pharmacokinetics of orbifloxacin after a single oral administration of 5.0 mg/kg body weight was evaluated in three dogs with experimentally induced chronic kidney disease (CKD group) compared to three healthy dogs (healthy group). The mean glomerular filtration rates by inulin clearance for the CKD group and the healthy group were 2.32±0.45 m<i>l</i>/min/kg and 4.14±0.66 m<i>l</i>/min/kg respectively. The maximum blood concentration of orbifloxacin was 3.60±0.23 μg/m<i>l</i> in the healthy group one hour after administration, and 3.07±0.31 μg/m<i>l</i> in the CKD group two hours after administration. The average blood concentration of orbifloxacin a 24 hours after administration was 0.45±0.14 μg/m<i>l</i> in the healthy group and 0.41±0.10 μg/m<i>l</i> in the CKD group. In this study the concentration of orbifloxacin were higher in the CKD group, whereas there was no significant difference compared to the healthy group. In conclusion, orbifloxacin showed favorable pharmacokinetic properties with no obvious adverse reactions in the CKD group compared to the healthy group.Therefore, these results suggest that no dosage adjustment of orbifloxacin is needed for CKD dogs.
