Abstract
Achievement of equity in access to health care is expected to reduce the social disparity in health. The aim of the study was to evaluate the inequity in access to health care focusing on the socio-economic barrier. We used 3,378 people data selected from the survey Japanese Study of Stratification, Health, Income and Neighborhood (JSHINE) whose study population was aged from 25 to 50 lived in Tokyo and its metropolitan area. Regarding temporary outpatient service, regular outpatient service, and inpatient service, we calculated the concentration index (CI) and the horizontal inequity index (HI) after adjusting the health need. As a supplemental analysis, we calculate CI for restraining from receiving medical services. Both the uses of temporary and regular outpatient services showed pro-rich distribution (temporary outpatient, 0.038 ; regular outpatient, 0.053). The use of inpatient service showed pro-poor distribution (CI : −0.032). HI showed pro-rich distribution in temporary outpatient service (HI : 0.043), regular outpatient service (HI : 0.063), and inpatient service (HI : 0.011). The supplemental analysis for restraining from receiving medical services showed the pro-rich distribution (CI : −0.064).
