Abstract

This study aimed to clarify the discharge supporting system of hospital and nursing practices associated with providing discharge support for patients who need medical treatment after discharge. <br>This survey was conducted in general hospitals in the A prefecture. The following findings were clarified: (1) 69.0% of the surveyed hospitals had departments that provided support to discharged patients (sections). (2) The structure of nursing practice in hospitals' units integrates factors such as finding case, coordination with a multidisciplinary health care team,observation of patients' condition after discharge, execution of prescribed medical treatment and care. (3) Nurses in the hospitals that had staffing of 7 nurses for a patient, sections, nurses in those sections, link-nurses, whole study meeting in hospitals practiced more discharge support than other hospitals. (4) Nurses in the units that could hold conferences with sections and had a conductive environment to facilitate exchange of information with visiting nurses or care managers and those who could monitor patients' situation after discharge practiced more effective discharge support than those in other units.
