Abstract

4歳時にマルファン症候群に伴う両側の水晶体偏位に対し水晶体再建術を行い,その後残余水晶体は瞳孔領から周辺部に偏位したが,約20年後に前房内へ脱臼した症例を経験した.摘出した再生水晶体では,Soemmering's ringが形成され,病理組織学的に赤道部に水晶体上皮細胞の増殖・分化部位であるlens bow形成が認められ,前後囊接着部でもlens bow形成を思わせる核の配列を認めた.また,前後囊接着部では,新しい水晶体囊形成と思われる細い線維性組織を認めた.偏位したSoemmering's ringでも,水晶体上皮細胞の増殖・分化が術後20年経過しても持続していると思われた.

We reported a case of dislocation of Soemmering's ring, which might have been located at peripheral portion behind iris, into the anterior chamber about 20 years after operation for dislocated lens. In pathological examinations, lens bow was recognized around equator area. The arrangement of nuclei around adhesion area of anterior and posterior capsule indicated a formation of lens bow. Moreover, the membrane, which showed the new formation of lens capsule, was recognized around adhesion area. These findings suggest that lens epithelial cells continue to grow and differentiate in the dislocated Soemmering's ring during about 20 years.
