Abstract
This is a case report of postoperative meningoceles that occurred in bilateral temporal bone. During the operation, we attempted to stop the discharge of the cerebrospinal fluid (CSF). The patient was a 47 year-old woman, who was referred to our clinic for operation of cholesteatomas in both ears. During the first operation for both ears, the brain dura mater was extensively exposed. In the second operation for the left ear, we found a cholesteatoma-like round shadow in the mastoid by preoperative CT scan, however which proved to be a meningocele. After replacing it, we discovered the leakage of the CSF from the tectorium of mastoid. It was repaired with the bony putty and fascia fixed by fibrin glue, resulting in a successful operation. Although CT scan revealed the same condition even in the right ear, we failed to observe any signs suggesting meningocele by MRI. Nonetheless, a meningocele was discovered in the right ear during the second operation. To repair the leakage, we put an absorbable poliglycolic acid sheet beneath the bony putty, fascia, cartilage and fixed them with fibrin glue. In addition, the mastoid cavity was filled with bone putty and a periosteum musculo flap. The patient has not experienced any particular issues even a year after the operation.
