Abstract

<p>【目的】近年24時間自由行動下血圧測定 (ABPM) の普及で, ABPMにより測定したambulatory arterial stiffness index (AASI) が, 動脈硬化や心血管イベント, 特に脳梗塞の指標としての有用性が評価されてきている. しかしながら血液透析 (HD) 患者におけるAASIの有用性については明らかでない. われわれはHD患者においてAASIが頸動脈末梢血管抵抗 (resistance index: RI) と関連するかについて検討した. 【方法】HD患者15名に対しHD前から24時間ABPMを行った. ABPMより算出したAASIと超音波ドップラー法で検査した頸動脈RI (総頸動脈: common carotid artery: CCA, 椎骨動脈: vertebral artery: VA) との関連について統計学的に検討した. 【結果】ABPMより算出したAASIは1.39±0.10と高値であった. CCA-RIとVA-RIは, それぞれ0.83±0.05, 0.75±0.09であり, AASIとそれぞれ有意に正相関していた (CCA-RI: R<sup>2</sup>=0.353, p=0.02, VA-RI: R<sup>2</sup>=0.474, p=0.005). 【結論】血液透析患者におけるABPMから算出されるAASIは頸動脈末梢血管抵抗を反映しており, 脳血管障害の指標となる可能性が示唆された. </p>

<p>【Background and Aim】Recently, it has been reported that in addition to being a predictor of atherosclerosis the ambulatory arterial stiffness index (AASI), a new ambulatory blood pressure parameter, also reflects cardiovascular disease. However, it remains unclear whether the AASI is associated with atherosclerosis in hemodialysis (HD) patients. The purpose of this study was to evaluate the association between the AASI and the carotid arterial resistance index (RI) in HD patients.【Methods】Fifteen HD patients were included in our study. All of the patients underwent ambulatory blood pressure monitoring (ABPM) and had their AASI measured. In addition, the common carotid artery-RI (CCA-RI) and vertebral artery-RI (VA-RI) were measured by ultrasonography, and the relationships between the AASI and RI were analyzed.【Results】The HD patients had much higher AASI than the normal subjects. Positive correlations were detected between the AASI and the CCA-RI or VA-RI (CCA-RI: R<sup>2</sup>=0.353, p=0.02 ; VA-RI: R<sup>2</sup>=0.474, p=0.005).【Conclusion】These results indicate that the AASI as measured by ABPM might be a useful predictor of the degree of carotid artery stenosis in HD patients.</p>
