Abstract

A case was 44-year-old male who had been having sex with males. He visited our hospital for complaints of fever and mucous and bloody stool, and was found to be suffering from amebic colitis. At that time, his serum hepato-biliary enzymes indicated normal levels and he was negative for serum hepatitis B surface antigen (HBsAg). Although amebic colitis was resolved with administration of metronidazole, general fatigue associated with elevated levels of serum hepato-biliary enzymes and positive conversion of HBsAg was emerged. He was diagnosed as having acute hepatitis B (AHB), because elevated level of immunoglobulin M-hepatitis B core antibody was detected. The genotype of HBV was genotype A. Thereafter, serum hepato-biliary enzymes decreased gradually, and normalized with HBsAg/anti-HBs seroconversion. It is suggested that careful observation and examination, taking incubation period into consideration, are important in clinical practice of sexually transmitted diseases.
