- Surgical Obliteration of Spinal Arteriovenous Fistulas Vascular Orientation by Selective Dye Injection Technique

<p> Objective : The most important point in the surgery for spinal vascular lesions is their complete obliteration while still preserving normal blood flow to the spinal cord. The authors focus on the utility of selective dye injection technique to assist in the direct surgery of spinal arteriovenous fistulas (AVF) or arteriovenous malformations (AVM).</p><p> Methods : Nineteen cases of spinal AVF or AVM have been treated over the past 9 years were included in our study. Selective dye injection technique has been introduced to obtain the vascular orientation during the surgery. A catheter was placed in the parent artery of the AVF or AVM before patient positioning. Indigo carmine (2 mg/ml) was injected selectively into the feeding arteries under the full exposure of the vascular lesion. Illustrative cases of spinal AVF were also presented.</p><p> Results : The selective dye injection technique appeared to be effective in determining the vascular orientation with blood flow dynamics. No side effects caused by dye injection itself were noted.</p><p> Conclusion : This selective dye injection technique can be applied without any special equipment, and may make the direct surgery of spinal AVFS or AVMS easier and safer.</p>
