Abstract

症例1は47歳,女性.主訴は右下腹部膨隆.腹部CTにて右腹直筋外縁に腹壁の欠損と脂肪組織の脱出を認めSpigelianヘルニアと診断し,TAPP法による腹腔鏡下ヘルニア修復術を施行した.症例2は71歳,女性.主訴は右下腹部膨隆.腹部CTにて右腹直筋の外側に腹壁の欠損と腸管の脱出を認めた.Spigelianヘルニアと診断しTAPP法による腹腔鏡下ヘルニア修復術を施行した.症例3は64歳,女性.主訴は右下腹部膨隆.腹部CTにて右腹直筋外側に腹壁の欠損と腸管の脱出を認めSpigelianヘルニアと診断し,TAPP法による腹腔鏡下ヘルニア修復術を施行した.いずれの症例も術後合併症は生じず,観察期間中の再発は認めていない.Spigelianヘルニアの腹腔鏡下手術の報告例は少なく,若干の文献的考察を加え報告する.

Case 1 involved a 47-year-old woman presented with a right lower abdominal bulging. Abdominal CT scan showed a defect in the abdominal wall at the lateral edge of the right rectus abdominis muscle and prolapse of fatty tissue from there. Spigelian hernia was diagnosed and we performed laparoscopic hernia repair by the trans-abdominal pre peritoneal (TAPP) approach. Case 2 involved a 71-year-old woman presented with a right lower abdominal bulging. Abdominal CT scan showed a defect in the abdominal wall at the lateral aspect of the right rectus abdominis muscle and prolapse of the bowel from it. Spigelian hernia was diagnosed and laparoscopic hernia repair by TAPP approach was performed. Case 3 involved a 64-year-old woman presented with a right lower abdominal bulging. Abdominal CT scan revealed a defect in the abdominal wall on the lateral aspect of the right rectus abdominis muscle and bowel prolapse through it. Spigelian hernia was diagnosed and laparoscopic hernia repair by the TAPP approach was performed. In all cases, no postoperative complications occurred and no recurrence was noted during observation period.<BR>Laparoscopic surgeries for Spigelian hernia have rarely been reported so far and we present our cases together with some bilbliographic comments.
