Abstract

<p>Physicians should be skilled in “dialogue” and “collaboration” when working in community-based integrated care systems. In order to care for further increasing chronic diseases, shifting to a “life model” of health care is critical. It would be an approach that integrates the patients’ narratives into a shared decision-making process (Patient-Centered Clinical Method), or an approach that treats the patient as a part of a family system and cares for patients/their families in the process of dialogue (Family-Oriented Care). Moreover, it is also essential for physicians to collaborate with the community as the partner and aim to solve problems (Community-Oriented Approach), or to facilitate an environment where multiple professionals can collaborate (Collaborative Leadership). Furthermore, “Salutogenic Approach” that strengthens the resources that support health, would be another vital approach for physicians working in the community.</p>
