Abstract

【目的】退院支援過程における退院調整看護師(以下DCN)とソーシャルワーカー(以下SW)が用いる,それぞれの職種に特有な専門性を反映した技術の性質や特徴を明らかとする。【方法】同一施設で働くDCNとSW5組10名を対象に退院支援実践について半構造化面接を実施し,その内容を質的帰納的に分析した。【結果】DCNとSWの退院支援の意図は共通であっても,家族支援,目標設定や課題のアセスメント,他職種との協働,院内の退院支援の質向上への働きかけ等において,用いる技術に専門性を反映した特徴や性質の違いが見られた。DCNは,療養者の予測される状態を自らアセスメントして他職種にその内容を確認し,SWは療養者の現在のニーズを多職種との連携・調整を行う中で把握するという特徴が明らかとなった。【結論】退院支援過程におけるDCNとSWが用いる技術の性質や特徴は「支援対象の捉え方」と「多職種との関り方」を中心に専門性が反映されることが明らかとなった。

【Purpose】The purpose of this study was to clarify the characteristics of Discharge Planning Skills for Discharge Coordinator Nurses(DCN) and Social Workers(SW).【Method】Semi-structured interviews were organized .Interview dates were collected from 5 DCNs and 5 SWs who were working in pairs at the same hospitals. All dates were analyzed by the qualitative inductive analysis method.【Findings】While the intent of discharge planning for DCN and SW were also the same, there were some differences in the characteristics of the skills. In particular, differences were evident with in the degree of family support, goal-setting, the process of assessment, the cooperation with other professionals and the method of improving the quality of the discharge support system in hospitals. It was clarified that DCNs confirmed their own judgement on the predicted status of the patient to other professionals within their field. SWs assessed the current needs if their clients and cooperated and coordinated with colleagues of varying professionals.【Conclusion】It was revealed that the expertise is reflected mainly characteristics of discharge planning skills for DCNs and SWs centered on "how to assess their clients" and "how to cooperated with other professionals ".
