Abstract

A 38-year-old female was seen at the hospital because of a painful pulsatile mass in the right palm, In June 1990, the patient got a bruise on the right palm by a bicycle accident, and two weeks later, she became aware of the pulsatile mass in the palm. In June 1991, she started to feel a pain in the region. On admission to the hospital the pulsatile mass with a diameter of about 10mm existed at the root of the second finger of the right hand. The pulsation disappeared when the radial artery was compressed. Angiography revealed an aneurysm at the bifurcation of the common palmar digital artery to the second and third finger palmar dygital artery. In December 1991, the three arteries were ligated and the aneurysm was excised. Pathologically it was a true aneurysm.<br> Fourteen cases of palmar aneurysm including this case have been reported in Japan. In many cases, blunt trauma contributed to the development of the aneurysm. It commonly occurred in the ulnar artery. Resection of the aneurysm with arterial ligation was the most requently employed procedure. It is emphasized here that the operative procedure should be followed by arterial reconstruction by microsurgical technique.
