We have experienced 2 cases of necrotizing fasciitis associated with longstanding uncontrolled diabetes mellitus.<BR>Case 1, a 67-year-old-man, who had a lesion on his left buttock, was treated with intravenous administration of antibiotics and insulin. From day to day pieces of slough were cut away but it continued to spread. On the 21st day, he died of DIC and mutiple organ failure.<BR>Case 2, a 48-year-old-woman, was admitted with complaining of intense pain on her waist of two weeks' duration. The skin lesion was aggravated in spite of treatment. On the 4th day, based on the diagnosis of necrotizing fasciitis, extensive surgical debridement was performed. The denuded area granulated rapidly, and so the wound was covered with mesh skin graft, which stuck successfully.<BR>From these experiences, the importance of early extensive surgical debridement in addition to antibiotic therapy was emphasized.
