A 20-year-old man visited our hospital for the first time on july 22, 1992. About three months before the visit, dark-red nodes protuberating in a hemispherical shape developed on the back of the left thigh and on the left lower leg. Since the nodes gradually grew accomponied with hemorrhage and tenderness, the patient visited the department. Histopathologically, proliferation of polymorphic histiocyte-like cells and proliferation of fibroblast-like cells were found in the entire layer of the dermis directly underneath the epidermis. Although inflammatory cell infiltration was marked, neither atypical giant cells with bizarre nuclei nor abnormal mitotic figures were observed. No obvious storiform pattern was seen in the fibroblast-like cells either. The peripheral region of the node was covered with a fibrous membrane, and therefore, the margin was distinct. In the immunohistochemical examination, the node was stained positive for vimentin, and negative for keratin, s-100 and desmin. On the basis of these observations, the lesions were diagnosed not malignant fibrous histiocytoma but atypical fibroxanthoma. Each node was resected together with the surrounding tissue of about 0.5 cm on top of the fascia. Since the surgery, no recurrence has been observed for two years and six months.
