Abstract

Extracranial carotid artery aneurysms are relatively rare. Recently, we treated two cases. Case 1 was in a 57-year-old woman who had had a mass of the right side of the neck for 20 years. She was referred to our hospital. No neurologic signs were present ; however, a pulsating neck mass, 2.5X2.5 cm in size, was found. No bruit was heard. Enhanced computed tomography showed a mass lesion in the right side of the neck. Magnetic resonance angiography (MRA) showed a saccular aneurysm. Case 2 was in a 46-year-old woman who had suffered dizziness. She was referred to our hospital for treatment of cerebellar hemorrhage. No neurologic signs were present, and a neck mass was not found. No bruit was heard.<BR>Cerebral angiography showed a saccular aneurysm of the internal carotid artery near the carotid bifurcation. However, both patients rejected operations. In Japan, extracranial carotid artery aneurysms are more rare than in other countries. These aneurysms often cause cerebral vascular dysfunction. Therefore, the need for surgical treatment has been stressed. Recently, operative indications have enlarged as surgical techniques have improved. In addition, intravascular surgery may be useful. We hope to establish techniques for such treatment.
