The provision of health care in the most cost-efficient way is examined here by focusing on the potential clash between the overall good of society as perceived by economists, and the doctor's primary duty towards the welfare of individual patients. Based upon a symposium held at Lisse in The Netherlands in September 1986, it examines differing attitudes to this problem and places them in legal and historical perspectives. Included are discussions of the problems of particular aspects of health care, such as medical expert systems, general practice, medical education, and clinical decision-making, where the direct involvement of doctors in allocating resources is most apparent.
